*spoilers*
Flemeth lives no matter what. If you didn't kill her in DA:O, then obviously she isn't dead. If you *did* "kill" her, well....she's still alive. I won't go into the details, but one of the required quests is basically meant to revive her. I suspect she'll play an even greater part in the next Dragon Age.
Her character design has been seriously remodeled. She looks much more like a Witch of the Wilds than some random, nasty old woman in a peasant dress.
