How to avoid the Wayward Son glitch in Dragon Age 2?

This might be old but I recently bought DA2 and ive heard about the Wayward Son glitch. For those who have experienced the glitch, how did you get passed it. Also it would be helpful if you knew how to avoid the glitch.
